POPULATION HEALTH STRATEGIES

The path forward is clear, right?
Health care is an unsustainable 18% of the United States' Gross Domestic Product and shows no sign of decreasing let alone moderating. American business and tax payers are literally going bankrupt while the value of American health care continues to lag by virtually every measure of global health care quality. The ACA, MACRA, CPC+, Medicaid waivers, etc... are the governments' market based response to health care transformation - expand coverage and engage providers to take greater accountability for improving the quality, cost and consumer experience (Triple Aim).

GAINING CLARITY
The transformation from volume to value occurs at the intersection of the financing (payer) and delivery (provider) of healthcare. Instead of considering these as two opposing forces, we believe there is untapped potential and transformational energy released when the two join forces and come into alignment.
How do you gain perspective and clarity around your population health vision and strategies? How do you establish a road map for meaningful change and transformation that is informed by the nuances of your market, your resources, your competitors, your payer mix and your leadership? What are the attributes of a good population health partner? What are the core competencies necessary to succeed? What comes first? How far do you need to go and how fast?

OUR APPROACH TO POPULATION HEALTH
Whether you are a physician group, hospital, health plan, or other health care provider or purchaser, each day you are facing a myriad of strategic decisions that will materially impact how successful you will be serving the needs of the community, now and in the future. The truth is, you can't expect to succeed on you own. It requires a strong partnership, not adversarial relationship, between payer, provider and consumer.

At Population Health Strategies we facilitate and support the payer and provider community to take increasing leadership and accountability for improving health of the community they serve. We build partnerships between payers and providers, providers and administration, and providers with providers.
